Please check your summons and letter for the date and time that you must appear in court for orientation.

COMPLETE JUROR QUESTIONNAIRE

This questionnaire must be completed prior to your first day of reporting for jury duty. If you are unable to complete it by that date, you will receive assistance during the juror orientation session.

COMPLETE EXCUSAL AFFIDAVIT

If you qualify to be excused for jury service, you must complete an affidavit and return it to the court. An affidavit was included in the mail with your summons. Alternatively, an affidavit with instructions on how to complete may be found on our FAQ page.

REPORT FOR SERVICE

Please arrive to the Carroll County Courthouse by 1:30PM on your report date to begin the jury selection process.
